We are happy to register new NHS patients within our catchment area.

How to Register

Please note we can only accept registrations between the times of 10am and 5pm

To register with our practice, please come into reception to get a form, or follow the link below to download the GMS1 New Patient Registration form, which you can print off and bring into the surgery;

After you have completed and handed in the form, you must attend the surgery with 2 forms of identification (ID).

Acceptable Identification; tenancy agreement, mortgage agreement, driving licence, passport, birth certificate. Utility bills credit card bill and bank statements must be dated within the last 2 months.

When you come in with your ID, please also print off the relevant patient questionnaire (based upon your age group), fill it in clearly using BLACK ink and bring it with you to the practice. We also have these forms available at reception.

Until you have submitted your ID you will not be registered at the practice.
